Hospice Care Team, Inc. is a non-profit community based agency providing hospice and bereavement services to individuals and their families diagnosed with a life-limiting illness or who have experienced a death of a family member or friend. The hospice services offer an interdisciplinary group of professionals (medical director, nurse practitioner, registered nurse, licensed vocational nurse, licensed social worker, chaplain, hospice aide, volunteers, and bereavement) that work with the patient/family to establish a plan of care and conduct visits per the plan of care. Other services include coverage of medications related to the hospice diagnosis, durable medical equipment (electric bed, bedrails, gel mattress, wheel chair, shower chair, bedside comode, walker, oxygen, and other equipment as ordered) , and medical supplies as ordered by the physician. A doctor's order is required to begin hospice services. Hospice services is offered to pediatric and adult populations. No one is denied hospice services based on ability to pay. HCT bereavement services is open to individuals in the community who have experienced a loss. For adults, individual and group counseling is available throughout Brazoria County. For children, individual and group counseling is available. HCT also offers Mother's Day Memories and Father's Day Memories for children that have lost a parent to death. Coping for the Holidays is offered to adults as a three week group session and for children as a half-a-day event. Annually, HCT sponsors Camp Erin - Houston for children to have a safe place for children to grief. No one turned away from HCT bereavement services due to inability to pay. Anyone can make a referral for bereavement services.
Yes, Hospice Care Team, Inc. is accepting volunteers. Our volunteer can be assigned to: 1) patient to provides periods of respite, reading to patients, listening to music, taking a ride, listening to life review, or enjoying in activities that patients enjoy as they are permitted to do. Volunteer may be asked to run errands, pick up medications, buy groceries to support the caregiver; 2) Pediatric patients to play with siblings, offered respite, run errands for parents, listen, and other activities to provide support; 3) Bereavement support to suviving family members under the direction of the Director of Bereavement Services; 4) Administrative Duties in the office; 5) Fundraising in the community; 6) Speaker's Bureau 7) Pet Therapy.
